This position focuses on quality assurance review, with a flexible schedule suited to an experienced home health nurse who is self-directed and meticulous. The ideal candidate is skilled in Axxess EMR and well-versed in Joint Commission standards.
Key Responsibilities:
1. Documentation Review & Compliance
Audit patient charts (SOC, recertification, discharge, PRNs) for completeness, accuracy, and regulatory compliance.
Verify that documentation supports homebound status, medical necessity, and skilled need.
Review and correct OASIS data to ensure accurate PDGM grouping and reimbursement alignment.
2. Axxess EMR Utilization
Use Axxess EMR to monitor documentation timeliness, track deficiencies, and generate QA reports.
Identify and address documentation trends or concerns across disciplines.
3. Joint Commission Accreditation Support
