Job description

Affinity is looking for reliable, compassionate, professional Registered Nurses (RNs) to care for our patients in Montgomery County. The ideal RN professional is highly skilled, organized, and compassionate. We have a variety of opportunities available (e.g., Shift Care, Per Diem, Start of Care (SOC), Skilled Visits, Hourly assignments, etc.) depending on your availability and preferences. We use Axxess and HHAexchange.

Responsibilities
  • Record and assess a patient’s medical history accurately
  • Monitor and record vital signs including blood pressure, temperature, heart rate etc.
  • Perform Start of Care (SOC) visits and ongoing patient evaluations
  • Develop and update patient care plans in coordination with physicians
  • Monitor patients’ condition and identify progress or sideeffects of treatments
  • Administer medications, treatments, and IV therapy as prescribed
  • Educate patients and families and provide emotional support
  • Maintain accurate documentation and ensure compliance with home health standards

  • Proven experience as a Registered Nurse
  • Ability to manage patient care independently and within a team
  • Familiarity with home health care standards and procedures
