CareGivers America - NEPA, a Medicare-certified Home Care company, is seeking a compassionate and skilled Registered Nurse to join our team as a Per Diem Pediatric RN Case Manager. Work Location: On the Road (Branch Location: 718 State St. Clarks Summit, PA 18411)
Territory: Schuykill County
Schedule: Per Diem | 1-2 days per week
- OASIS Start of Care (SOC): $120
- OASIS Resumption of Care / Recertification: $100
- Routine / Follow-Up Visits: $60
- Mileage reimbursement provided

As a Pediatric RN Case Manager, you will:- Complete comprehensive patient assessments, including OASIS data collection, within established timelines
- Collaborate with physicians, healthcare professionals, patients, and families to develop individualized pediatric care plans
- Provide direction and supervision to LPNs and Home Health Aides to ensure quality care
- Monitor clinical documentation for accuracy, timeliness, and regulatory compliance
- Communicate regularly with patients' families and care teams to coordinate services
- Participate in quality improvement initiatives and staff education
- Promote a safe and respectful care environment for patients and colleagues
- Active PA RN License (Required) - BSN Preferred
- 2+ years of professional nursing experience (pediatric or home health experience preferred)
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities and work independently
- Valid driver's license, and current CPR certification
