Job description
- Evaluate and admit patients to home care; develop plan of care and provide care to patient according to plan.
- Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs.
- Review and plan patient care and or discharge during scheduled case conference meeting and document accordingly.
- Act as liaison for patient/caregiver with MD and community healthcare partners.
- Strive to lower cost of care through managing mileage, supplies and visit scheduling.
- Completes all documentation related to patient contact, assessments, SOC, and recerts.
- Resumption of care and discharge according to policy.
- Maintain understanding of current home care regulatory guidelines.
- Provide on-call coverage for both home care and hospice as assigned.
- Demonstrate and encourage team behavior and exceptional patient/guest experiences.
- Uphold and promote patient safety and quality.
Education:
Associate's degree in nursing and currently licensed by the state of Alabama required.
Experience:
Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER. Previous home care or hospice experience preferred.
Additional Skills/Abilities:
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize a schedule to meet patient needs.
