Position Overview Admissions RN Connected HH is seeking a full‑time, hourly Home Health Admissions RN to support our Portland Metropolitan service area. This role is ideal for nurses who excel in first‑visit assessments, care‑plan initiation, and establishing a strong foundation for each patient’s home‑health episode. Key Responsibilities Admissions RN Responsibilities
  • Complete comprehensive Start‑of‑Care (SOC) nursing assessments in patient homes.
  • Initiate individualized care plans and establish expectations for the home‑health episode.
  • Conduct medication reconciliation, safety assessments, and disease‑specific education.
  • Coordinate care with interdisciplinary team members to ensure timely service initiation.
  • Provide skilled nursing interventions as needed during SOC visits.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Communicate effectively with physicians, caregivers, and internal team members.
  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Qualifications RN Requirements
  • Current Oregon RN license (or ability to obtain).
