Job Description
We are seeking a dedicated and skilled Speech Therapist I to join our team. This position involves providing speech therapy services to patients in their homes, ensuring the highest level of care and support.
Key Responsibilities
  • Conduct assessments and develop individualized treatment plans for patients.
  • Deliver speech therapy interventions and monitor patient progress.
  • Document patient care and progress in compliance with established guidelines.
  • Travel to patient homes for visits, providing care as scheduled.
  • Participate in periodic office meetings and team collaboration.
Qualifications
  • Minimum of 1 year of recent adult home health experience is required.
  • Current licensure as a Speech Therapist.
  • Certifications: CPR and BLS required.
  • Proficiency in using HomeCare HomeBase EMR and OASIS is necessary.
  • Must possess reliable transportation, a valid driver's license, and agency-required liability insurance.
Travel Requirements
Employees must be willing to travel up to 75 miles from their designated home clinic to provide care for patients. Mileage will be reimbursed at the standard IRS rate.
