We're hiring for a full-time Family Nurse Practitioner (FNP) to join our Health Solutions Family Medicine/Marc facility.

What You'll Be Doing

As an FNP with Health Solutions Family Medicine/Marc, we are seeking a compassionate and skilled Family Nurse Practitioner (FNP) to join our multidisciplinary team. This role is central to delivering integrated, patient-centered care in alignment with PCMH principles, APM performance metrics, and MAT protocols. The FNP will provide comprehensive primary care services to individuals and families across the lifespan, with a focus on chronic disease management, behavioral health integration, and substance use disorder treatment. Creating and sustaining positive, compassionate interactions with patients and providing high quality care will be central to the work you do. Your attention to detail and problem-solving skills will be important to your responsibilities related to evaluating and managing medications.

RESPONSIBILITIES: As an FNP under the administration of the Director of Family Medicine Montebello and Marc Medical you will.

  • Maintain accurate, timely and current records with 100 percent compliance with Health Solutions' standards
  • Use electronic health records and electronic prescribing accurately and facilely to promote patient care
  • Conduct comprehensive health assessments, physical exams, and diagnostic evaluations for patients with kindness and a focus on their needs.
  • Diagnose and manage acute and chronic conditions using evidence-based guidelines.
  • Deliver care consistent with PCMH principles: whole-person orientation, coordinated care, enhanced access, and quality/safety improvement.
  • Participate in quality improvement initiative and use data to drive performance on APM measures such as patient engagement, preventive screenings, and chronic disease outcomes.
  • Engage patients in shared decision-making and self-management support.
  • Prepare and administer prescribed medications, injections, and vaccinations.
  • Prescribe medications and therapies, including buprenorphine and other MAT agents, in accordance with federal and state regulations.
  • Monitor patients through all phases of MAT: induction, stabilization, and maintenance.
  • Facilitate timely and accurate medication prescriptions and refills.
  • Maintain medications securely.
  • Review laboratory results with the treatment team and patients.
  • Apply best practices in medical care and medication assisted treatment.
  • Offer psychoeducation to patients and their family about topics relevant to high quality care (e.g., disease processes, sleep hygiene, nutrition, behavior modification).
  • Implement preventive medical interventions in collaboration with patients and healthcare specialists.
  • Evaluate behavioral health assessments and makes referral to behavioral health professionals.
  • Maintain productivity.
  • Provide coverage for other providers including but not limited to, acute visits, medication refills, and lab reviews.
  • Other duties as assigned.

Care integration and collaboration

  • Participate in and effectively collaborates with our multidisciplinary teams of professionals to include behavioral health clinicians, care coordinators and pharmacists to ensure continuity of care.
  • Participate as a valuable team member in daily huddles, case conferences and care planning meetings.

Physical Requirements

  • Ability to stand and walk for extended periods (typically 8–12 hours), frequently moving between exam rooms and workstations.
  • Capacity to lift or move equipment and assist patients with repositioning, transferring, or supporting during examinations. Tasks may involve moderate weight (approximately 25–100 lb), sometimes requiring team assistance.
  • Frequent bending, stooping, twisting, and reaching—both overhead and below waist level (e.g., retrieving supplies) and at or below shoulder level (e.g., handling instruments)—including maintaining awkward positions for several minutes.
  • Ability to perform precise procedures such as blood draws, injections, suturing, and operate medical instruments/equipment or electronic systems.

Qualifications

What we're looking For—The Must-Haves

  • Current license as a Family Nurse Practitioner in Colorado
  • Graduate degree in nursing from an accredited academic institution
  • Two years of experience as an FNP
  • Computer proficiency required

  • Experience caring for individuals with addictions, and severe and persistent mental illness
  • Track record of working successfully as a part of a multidisciplinary team; experience in managed care environments
  • Familiarity with APM metrics and quality reporting
  • Experience in rural or underserved settings
  • NextGen (EHR) experience
  • Bilingual or multilingual abilities
  • Current Colorado driver's license and access to reliable transportation during work hours
