Job Summary

  • The Home Health Aide is responsible for providing personal care and simple rehabilitative/restorative services to clients according to written instructions prepared by the Home Health RN or Physical Therapist and carried out under the supervision of the appropriate health professional. The Home Health Aide provides care and service to persons of all age groups in their home or alternate site.

General Hours of Work

  • Monday through Friday; weekend & evening rotations

Education and/or Experience

  • High School diploma or GED preferred. Experience as a Certified Nurse's Aide or Home Health Aide preferred. Bi-lingual experience preferred but not required.

Certificate, Licenses, Registrations

  • Certified as a Nursing Assistant in the state of Iowa if providing services to a Medicare qualifying client. Must obtain CPR certification within 3 months of hire date. Access to automobile required. This position requires a valid driver's license and must carry their own automobile liability insurance. Telephone/cell phone accessibility recommended.

Essential Functions

  • Performs client care tasks such as: takes vital signs, I & O, weights and reports abnormalities to the Home Health RN or Manager; provides personal care and bathing as ordered; dresses and undresses clients; provides simple rehabilitative/restorative care as instructed by the primary therapist; provides bowel/bladder care as instructed by the Home Health RN; assists client with activities of daily living, including basic range of motion in order to increase physical mobility.
  • Facilitates the client's self-care at home and encourages client/family participation; provides assistance with client's self-administration of medications; provides care according to age specific needs; and maintains a clean and safe environment.
  • Follows the care plan as developed by the Home Health RN or therapist and communicates to the Home Health RN/therapist changes in the status of the client.
  • Assist with feeding, basic meal planning, preparation and purchasing of food. Perform routine daily life activity duties as identified on the plan of care and documents.
  • Completes required continuing education hours per year and attends care conferences regularly.
  • Able to work flexible hours and accept work assignments readily.
  • Performs other duties and responsibilities as assigned.
