What You’ll Do as a Physical Therapist:

  • Provide physical therapy services in accordance with applicable law, physician orders, payer requirements, and the authorized plan of care.

  • Evaluate patients in their homes and provide skilled physical therapy interventions to support mobility, safety, strength, balance, functional independence, and quality of life.

  • Develop, update, and implement care plans consistent with clinical judgment, physician orders, payer requirements, and applicable professional standards.

  • Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education as clinically appropriate and legally permitted.

  • Communicate with the referring agency, physician, patient, family, PTA, or other authorized care participants as clinically appropriate and legally permitted.

  • Document patient progress accurately and efficiently in accordance with applicable payer, agency, and regulatory requirements.

  • Current California Physical Therapist / PT license

  • Reliable transportation and valid driver’s license
