RESPONSIBILITIES:
  • Assess the psychosocial status of patients/families/caregivers related to the patient's illness and environment
  • Carry out social evaluations and plan interventions based on evaluation findings
  • Counsel patient/family/caregivers as needed in relationship to stress and other identified coping difficulties
  • Maintain clinical record on all patients referred to social work
  • Provide information and referral services for organization patient/family/caregivers regarding practical and environmental needs
  • Maintain collaborative relationships with organization personnel to support patient care
  • Maintain and develop contacts with public and private agencies as resources for patient and personnel
  • Participate in discharge planning when needed

REQUIREMENTS:
  • License in Social Work preferred
  • Experience in Long Term Care preferred
