Job description

Field Nurse (LPN)

Senior Helpers of Northern Queens is looking for a dedicated and compassionate Field Nurses (LPN) to provide support in our clients’ homes to help maintain their independence. The ideal candidate should be able to work independently and have a pleasant and friendly personality. We provide a full range of superior home care services to culturally diverse clients—Chinese, Korean, Spanish, Russian, Yiddish and English speaking clients.

Job Types: Full-time and Part-time Position Available

Classification: Non-Exempt

Pay: $35.00 per hour, plus $100.00 referral bonus tied to number of clients signed up for services and HHAs/Caregivers hired

Work Location: In Office/Remote/In Client’s homes, as needed. 

All Field Nurses will be required to submit or undergo a full annual physical with MMR (MEASLES/MUMPS/RUBELLA) and PPD/TB TEST(less than a year old) as required by the state.

ESSENTIAL DUTIES & RESPONSIBILITIES include but are not limited to:

  • Complete necessary documentation for nursing assessments and planning evaluation pertaining to clients and submit to Staff Registered Nurse (RN) for approval.
  • Work with RN to evaluate clients as necessary to ensure client needs are consistent with the care plan. 
  • Utilize professional assessment skills to identify physical, mental, and psychosocial needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary approach. 
  • Coordinate with management and Staff RN on implementing and revising operational procedures related to quality client care. 
  • Function as a resource and mentor to peers and subordinates to maintain professional nursing standards. 
  • Act on client’s behalf to assure patient rights. 
  • Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitudes and practices displayed by other staff members. 
  • Respond promptly to any client complaints concerning a caregiver, document complaints, and review complaints with the Staff RN and Director of Operations. 
  • Lead caregiver trainings onsite at Senior Helpers of Northern Queens office, as needed.
  • Manage Caregivers and provide feedback and training, as needed. 

ADDITIONAL RESPONSIBILITIES 

  • Complete all paperwork in a timely manner. 
  • Report any significant client changes to the office immediately. 
  • Maintain current certifications, health requirements and eligible working status as defined by the Department of Health and the Department of Homeland Security at all times. 
  • Follow all policies and procedures as outlined in the Employee Handbook for Field Staff. 
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS

  • High school diploma. 
  • Must be a licensed Registered LPN.
  • At least two years of experience working in home health care. 
  • Must meet NY state requirements for Caregiving/RN services
  • Must have safe and reliable transportation to go to and from the client’s location and/or facilities and have a vehicle available during assignments in order to assist clients with shopping needs if required.
  • Must be available to work various shifts, including nights, weekends, and holidays.
  • Preferred: Have a valid and current state driver’s license and insurance.
