Matrix Home Care is seeking an experienced Registered Nurse (RN) to provide hi-tech skilled nursing visits to patients in the Broward County area.
This is a per diem opportunity for an RN who is comfortable providing skilled care in the home setting, including advanced nursing services.
Position Summary:
The Registered Nurse (RN) will provide skilled nursing visits in patients' homes, complete clinical assessments, administer ordered treatments, document patient progress, and communicate with the care team to ensure quality patient care.
Responsibilities:
Provide skilled nursing visits in the home setting
Complete patient assessments and monitor changes in condition
Administer medications and treatments according to physician orders
Provide wound care and other hi-tech nursing services as needed
Educate patients and caregivers on treatment plans, safety, and disease management
Document visits accurately and timely in the EMR system
Communicate with physicians, case managers, and clinical leadership as needed
Follow agency policies, state regulations, and standards of care
Requirements:
Active Florida Registered Nurse (RN) license
Home health experience preferred
Experience with skilled nursing visits
Strong assessment, documentation, and communication skills
Reliable transportation, valid driver's license, and auto insurance
Current CPR/BLS certification
Ability to work independently in the field
Successful AHCA Level 2 background screening
