Job description

Join the Amarillo VA Medical Center as Home-Based Primary Care (HBPC) Medical Director

The Amarillo VA Medical Center, part of the Thomas E. Creek VA Health Care System, is seeking a dedicated and experienced physician to serve as the Home-Based Primary Care (HBPC) Medical Director within our Geriatric and Extended Care (GEC) Service.

This is a unique leadership opportunity to shape and advance a vital program serving our nation's Veterans-providing comprehensive, interdisciplinary care to frail and chronically ill patients in the comfort of their homes.

Position Overview

The HBPC Medical Director serves as the senior clinical leader for the HBPC program, overseeing care delivery across Amarillo and affiliated satellite locations. This role combines leadership (51% or more administrative FTE) with direct patient care (up to 49% clinical FTE) across GEC services, including outpatient geriatrics, Community Living Center (CLC), and Hospice & Palliative Care.

You will collaborate closely with the HBPC Program Director and report to the Chief of Geriatric and Extended Care.

Key Responsibilities

Leadership & Program Oversight

  • Provide strategic and clinical leadership for the HBPC program

  • Collaborate with program leadership on operations, staffing, and performance improvement

  • Ensure compliance with VHA, Joint Commission, and accreditation standards

  • Lead program development and advocate for HBPC services within the VA and broader medical community

  • Participate in audits, reviews, and accreditation processes

Clinical Excellence

  • Deliver comprehensive primary care to medically complex Veterans in home settings

  • Supervise and collaborate with interdisciplinary team members, including advanced practice providers

  • Oversee patient care plans, consultations, hospital admissions, and transitions of care

  • Perform evaluations, diagnostics, prescribing, and ongoing management of acute and chronic conditions

Education & Collaboration

  • Teach and mentor medical students, residents, and fellows

  • Partner with VA specialists and community providers to coordinate care

  • Promote best practices and advancements in geriatric and home-based care

Program Operations

  • Participate in team meetings and ensure effective communication across staff

  • Oversee patient safety initiatives, chart reviews, and quality monitoring

  • Coordinate physician coverage and ensure continuity of care

  • Support caregiver and patient education efforts

Ideal Candidate

  • Board-certified physician with expertise in geriatrics, internal medicine, or family medicine

  • Strong leadership and team-building skills

  • Passion for caring for complex, aging populations

  • Experience with interdisciplinary care and home-based services preferred
