Location: Montgomery County, MD, Washington, DC, Frederick County, Howard County, Prince George's County, or Northern Virginia
Schedule: Part-Time ranging between 20-25 hrs weekly

About the Role

We are seeking an experienced, compassionate, and highly professional Senior Care Consultant to join our team. This role is ideal for someone who is passionate about supporting older adults and vulnerable populations while helping individuals and families navigate complex healthcare, community, and long-term care systems.

Our Senior Care Consultants serve as trusted advisors and advocates, helping clients maintain independence, access appropriate resources, and achieve the highest possible quality of life. This position combines client advocacy, care coordination, problem-solving, and relationship management.

  • Job Duties:
  • Conduct an in-home or virtual in-home assessment for each new client and perform a comprehensive biopsychosocial assessment.
  • Develop a care management (CM) "Plan of Care" in conjunction with Director of Care Management and the client/family.
  • Create a "Recommendations Report" based on the client assessment, clients' needs and wishes - present this to the client, family members and/or professionals.
  • Make follow up home visits (in person or virtually) to the client to coordinate, implement, modify, & evaluate the Plan of Care activities and to assess safety, health or cognitive risks.
  • Act as an advocate and liaison to medical providers & other health care team members.
  • Escort and drive clients to physician appts & other appts as needed.
  • Provide supportive counseling/family support/mediator services as needed.
  • Provide assistance/guidance to (or interaction with) guardianship/conservatorship as needed.
  • Provide collaboration/assistance with or for any fiduciary needs as needed.

Job Qualifications and or Educational Background

We welcome candidates from a variety of professional backgrounds who have demonstrated success working with older adults or other vulnerable populations, including children, individuals with intellectual disabilities, and those requiring long-term support services.

Relevant Educational and Professional Backgrounds May Include:

  • Gerontology
  • Sociology
  • Psychology or Counseling
  • Occupational Therapy (OT)
  • Physical Therapy (PT)
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Social Work (LCSW-C, LICSW, MSW, etc.)
  • Healthcare Facility Administration
  • Recreational Therapy
  • Senior Living Leadership
  • Nonprofit Leadership Serving Older Adults
  • Home Health or Home Care Management

Equivalent experience may be considered in lieu of a specific degree. For example, candidates who have held leadership positions within senior-serving organizations, memory care programs, villages, or community-based aging services may be excellent fits.

Required Experience

We are looking for candidates with substantial professional experience, not simply credentials or licensure.

Strong candidates will possess:

  • Multiple years of experience working directly with older adults or other vulnerable populations
  • Understanding of care management principles and client advocacy
  • Knowledge of the senior care continuum, including:
    • Independent Living
    • Assisted Living
    • Home Health
    • Rehabilitation Hospitals
  • Skilled Nursing Facilities
  • Aging in Place services and challenges
  • Experience coordinating services, supporting families, and navigating complex care situations

Preferred Experience

  • Home health, home care, or other in-home service experience
  • Experience conducting home visits
  • Experience working across multiple healthcare and senior living settings
  • Care coordination or case management experience

While much of the work can be performed remotely or from a home office, candidates must have reliable transportation and be willing to travel to client homes and community locations as needed.
