The Prime Care Wellness Institute is seeking a dedicated and experienced Physical Therapist to join our team at our Eugene, Oregon Rehabilitation Center. In this full-time on-site role, you will play a critical part in enhancing the physical well-being of our residents, primarily focusing on seniors. You will design, implement, and evaluate individualized therapy plans aimed at improving mobility, reducing pain, and promoting overall health.

As a Physical Therapist in our Rehabilitation Center, you will work in a dynamic setting that encourages collaboration among nurses, caregivers, and other healthcare specialists. • Conduct comprehensive physical therapy assessments to evaluate each resident’s mobility, strength, and functional capabilities.
• Develop personalized treatment plans based on assessment findings, targeting improvements in mobility, balance, and daily living functions.
• Implement therapeutic interventions including exercises, manual therapy techniques, and appropriate use of assistive devices.
• Utilize pain management strategies and establish fall prevention protocols to optimize resident safety and outcomes.
• Monitor resident progress meticulously, adjusting therapy recommendations based on continuous evaluations and documentation.
• Provide education to residents and their families regarding rehabilitation protocols and home exercises to ensure sustained progress.
• Collaborate closely with fellow healthcare providers, including nursing staff and physicians, to ensure integrated care delivery and optimal recovery paths for residents.
• Maintain thorough, accurate clinical documentation in compliance with all relevant healthcare regulations and practices.

Your primary focus will be on senior residents, many of whom may be recovering from surgery or managing chronic conditions. You will engage with this population by applying your extensive knowledge of geriatric therapy practices, ensuring that their unique physical needs and goals are met compassionately and effectively.

• A Doctorate (DPT) or Master’s degree in Physical Therapy from an accredited institution.
• An active Physical Therapy license in Oregon is required.
• Demonstrable experience in geriatric care or assisted living facilities is preferred, showcasing an understanding of unique challenges faced by senior patients.
• Strong foundational knowledge of physical therapy methodologies including mobility enhancement techniques and comprehensive pain management.
• Exceptional interpersonal communication skills to effectively connect with senior residents and their families.
• Ability to work autonomously whilst also valuing a team approach to patient care.
• Familiarity with electronic health record systems and documentation standards is essential.

• 2 years or more of clinical experience within a rehabilitation or assisted living environment, particularly with geriatric populations.
• Experience in developing and implementing fall prevention strategies and mobility enhancement programs.
• Involvement in community outreach or wellness programs targeting senior health improvement.
