Position Overview

The Prime Care Wellness Institute is seeking a dedicated and experienced Physical Therapist to join our team at our Eugene, Oregon Rehabilitation Center. In this full-time on-site role, you will play a critical part in enhancing the physical well-being of our residents, primarily focusing on seniors. You will design, implement, and evaluate individualized therapy plans aimed at improving mobility, reducing pain, and promoting overall health.

Key Responsibilities
  • Conduct comprehensive physical therapy assessments to evaluate each residentโ€™s mobility, strength, and functional capabilities.
  • Develop personalized treatment plans based on assessment findings, targeting improvements in mobility, balance, and daily living functions.
  • Implement therapeutic interventions including exercises, manual therapy techniques, and appropriate use of assistive devices.
  • Utilize pain management strategies and establish fall prevention protocols to optimize resident safety and outcomes.
  • Monitor resident progress meticulously, adjusting therapy recommendations based on continuous evaluations and documentation.
  • Provide education to residents and their families regarding rehabilitation protocols and home exercises to ensure sustained progress.
  • Collaborate closely with fellow healthcare providers, including nursing staff and physicians, to ensure integrated care delivery and optimal recovery paths for residents.
  • Maintain thorough, accurate clinical documentation in compliance with all relevant healthcare regulations and practices.
Patient Population / Client Focus

Your primary focus will be on senior residents, many of whom may be recovering from surgery or managing chronic conditions. Qualifications
  • A Doctorate (DPT) or Masterโ€™s degree in Physical Therapy from an accredited institution.
