This position is primarily onsite at our outpatient office within the assisted living facility, allowing you to work closely with residents in an encouraging and supportive setting. You will follow a structured work schedule from Monday to Friday, between 9 AM and 5 PM, which promotes work-life balance while ensuring residents receive consistent care. Our facility is designed to promote accessibility and encourage mobility, allowing Physical Therapists to utilize both communal spaces and private treatment rooms effectively.

• Conduct comprehensive assessments to evaluate residents' physical capabilities, limitations, and therapy needs.
• Design and implement individualized treatment programs aimed at improving mobility, reducing pain, and enhancing functional performance.
• Deliver hands-on therapy sessions using advanced techniques and modalities tailored to each resident's specific goals.
• Maintain thorough documentation of residents' progress, adjusting treatment plans as necessary based on ongoing evaluations.
• Collaborate with a multidisciplinary team, including physicians, occupational therapists, and nursing staff, to develop cohesive care strategies for residents.
• Educate residents and their families about recommended exercises and techniques to encourage continued progress and independence.
• Manage a typical patient volume of about 7 residents per day, ensuring personalized attention and quality of care.

You will primarily work with elderly residents, many of whom may have complex medical histories and various degrees of mobility challenges. Your expertise will guide seniors through rehabilitation processes after surgeries, strokes, or falls, focusing on restoring their functional abilities and enhancing their overall strength and stability. This role requires a compassionate approach to care, emphasizing patience and understanding as you assist residents in achieving their personal goals.

• A valid and active Physical Therapist (PT) license in the state of Oregon.
• A Bachelor’s degree in Physical Therapy is required; a Master’s or Doctorate Degree in Physical Therapy will be preferred.
• Strong communication skills to effectively interact with residents, caregivers, and team members.
• A patient-centered approach that emphasizes empathy and respect for all residents.
• Ability to work independently and as part of a collaborative healthcare team.

• A minimum of 2 years of prior experience as a Physical Therapist, preferably within an assisted living or rehabilitation setting.
• Experience developing treatment plans and executing physical therapy modalities specific to geriatric populations.
• Familiarity with electronic health record (EHR) systems to document progress effectively and communicate with team members efficiently.
