Job Title: Registered Nurse - Home Health
Facility: Providence Home Health LA County North
Coverage: Santa Clarita, CA / Surrounding areas
Contract: 13 weeks
Shift: 8:00 AM - 4:30 PM
Pay Rate: $63 per hour
Mileage Reimbursement: IRS Standard Rate
 

Providence Home Health LA County North is seeking an experienced and compassionate Registered Nurse (RN) to provide skilled in-home care to patients in the Santa Clarita, CA region and surrounding areas. The RN will work independently in the field, managing a caseload of home health patients, and is responsible for delivering safe, effective, and patient-centered care.
This role is ideal for clinicians who are self-motivated, organized, and skilled at managing diverse patient needs in a home setting.
 
Key Responsibilities:
  • Deliver direct nursing care in patients' homes including assessments, treatments, wound care, and medication administration.
  • Utilize OASIS documentation for accurate and timely clinical charting in EPIC EMR.
  • Visit up to 6 patients per day, meeting the standard 30 productivity points.
  • Collaborate with interdisciplinary teams to develop, update, and evaluate individualized care plans.
  • Educate patients and families on disease management, medication, and safety practices.
  • Maintain clinical compliance with state regulations and organizational standards.
  • Travel within a service area radius of up to 50+ miles with minimized windshield time.
