Job Title: Intake Coordinator
Facility: Curran-Fromhold Correctional Facility
Address: 7901 State Rd., Philadelphia, PA 19136
Contract: 13 weeks
Shift: 7:00 PM - 7:30 AM (36hrs/Week)
Pay Rate: $23/hr (W2)
Potential to float to other sites on campus based on operational needs!
60 minute lunch break.
Job Description:
As an Intake Coordinator at Curran-Fromhold Correctional Facility through YesCare, you will play a vital role in facilitating patient access to medical and behavioral healthcare services within a correctional setting. This position requires excellent organizational, communication, and clinical support skills, as well as a professional approach to maintaining both patient confidentiality and facility protocols.
Key Responsibilities:
  • Manage daily processes related to the documentation of patient appointment refusals.
  • Conduct hourly rounds in the intake area to monitor patient flow and report barriers to site leadership and custody staff.
  • Collect and manage data related to access-to-care initiatives as assigned.
  • Assist in coordinating intake assessments and ensure timely entry of patient demographic and medical information.
  • Communicate effectively with medical and correctional teams to ensure smooth coordination during the intake process.
  • Maintain accurate records in the electronic health record (EHR) system in compliance with HIPAA and facility policies.
  • Support security protocols and safety procedures in accordance with correctional facility guidelines.
  • Float to other intake units or departments as operationally required.
Experience & Requirements:
  • High school diploma or equivalent required.
  • Minimum of one (1) year of experience in a healthcare environment.
  • Active hands-on BLS certification from the American Heart Association or American Red Cross.
  • Strong communication, time management, and organizational skills.
  • Experience working in secure or correctional healthcare environments is a plus.
