Job description

A leading Federally Qualified Health Center just southwest of Rutland, Vermont is seeking an experienced Nurse Practitioner or Physician Assistant to provide high-quality primary care across multiple community-based clinic locations while enjoying a collaborative team environment and an exceptional work-life balance.
Position Highlights:

  • Provide comprehensive primary care across multiple clinic locations
  • Manage acute, chronic, and preventive care for patients of all ages
  • Perform comprehensive exams, diagnoses, and treatment planning
  • Order and interpret labs and diagnostic testing
  • Perform office procedures, including suturing and wound care
  • Prescribe medications and provide patient education
  • Coordinate care with physicians and multidisciplinary teams
  • Utilize integrated behavioral health and care management resources
  • Practice within a Level III Patient-Centered Medical Home model
  • Fully integrated EMR with strong clinical support
  • 0.9 FTE with a 4-day work week
  • Limited call averaging just 3 weeks per year


Qualifications:

  • Nurse Practitioner or Physician Assistant
  • Graduate of an accredited NP or PA program
  • Active Vermont licensure or ability to obtain
  • 3–5 years of primary care experience preferred
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and teamwork abilities
  • Commitment to high-quality, patient-centered care
