Job description

Responsibilities
Social Workers/Therapists are responsible for conducting therapeutic services for patients, interviewing and developing the psychological history, assessing the therapeutic needs of the individual and family, developing appropriate treatment objectives, and treatment plan updates for patients. Social workers/Therapists conduct family sessions prior to discharge. Social Workers/Therapists act as the patients' and families' primary contact during the patient's stay at the facility, as well as their advocate with the treatment team. Social Workers/Therapists are also liaisons with outside agencies and individuals for the purpose of developing and coordinating discharge planning with the treatment team and family. Any other duties as assigned.
Completes psychosocial assessments as assigned.
Conducts general psychotherapy group sessions for patients' at least three (3) times per week.
Demonstrates a high level of clinical skill in assessing the clinical needs of the patient, determining appropriate group interventions and in conducting the group in accordance with these needs.
