The Job
The LVN/LPN provides direct, onsite and remote care to Embrace members residing in assisted living and community-based settings under the direction of Nurse Practitioners and Physicians. This role focuses on focused assessments, wound care, vaccination support, IV therapy within scope, catheter care, and strong collaboration with community staff.
The LVN serves as a visible, professional brand ambassador and clinical extension of the primary care team, supporting high-quality care delivery and reinforcing Embrace workflows within assigned communities.
You Will
Clinical Care & Assessment
  • Round routinely at assigned communities as determined by need.
  • Check in with community staff upon arrival to identify urgent needs or triage concerns
  • Perform focused assessments including:
    - Vital signs, pain, mental status, and basic cardiopulmonary/GI observations
    - Skin integrity and wound status
    - Catheter function and output
  • Identify changes in condition and escalate appropriately

Direct Clinical Interventions (Within LVN Scope)
  • Administer vaccinations and IM/SC injections as ordered and per protocol
  • Perform wound care as directed, including dressing changes and monitoring
  • Perform routine foot care and maintenance.
  • Perform urinary catheter care and replacements per policy
  • Provide peripheral IV insertion, hydration, maintenance, and discontinuation with approved competencies
  • Monitor for adverse reactions, complications, or lack of response and escalate promptly

Transitions of Care & Medication Support
  • Conduct follow up check ins after transitions or acute events in urgent cases as directed by clinical team.

Communication & Care Coordination
  • Communicate frequently and proactively with the primary care team
  • Provide non urgent POA/family updates as directed
  • Build strong collaborative relationships with community staff, residents, and families

Community Engagement & Relationship Development
  • Maintain a consistent, visible presence within assigned communities
  • Partner with community leadership to identify needs and opportunities
  • Support member growth, retention, and overall satisfaction
  • Share field insights to inform care delivery and expansion efforts

All care is delivered under NP/MD direction and within applicable state and organizational regulations
  • Other duties as assigned.
  • Actively support the achievement of SCAN's Vision and Goals.

Your Qualifications
  • Licensed Vocational Nurse (LVN)
  • Associate's Degree or equivalent experience preferred
  • 3-5 years of related experience
  • Performs work under direct supervision. Handles basic issues and problems and refers more complex issues to higher-level staff. Possesses beginning to working knowledge of subject matter.
