Safeway Home Healthcare isseekinga compassionate, organized, and dependable Individualized Home Supports (IHS) Worker with Training to provide direct support services to individuals with cognitive and mobility-related disabilities while also serving in a leadership and mentorship role foradditionalIHS staff and clients.
This position combines hands-on community support, skill development, organization, and administrative coordination. The ideal candidate is proactive, highly organized, comfortable working independently, and passionate about helping individuals build independence, confidence, and meaningful community connections.
Reliable transportation isrequired, as this role involves regular community outings and support across multiple locations.
Primary Responsibilities
Direct Support Services Seward Client (Approximately 15 Hours/Week)
Provide direct IHS services to one primary clientlocatedin the Seward neighborhood of Minneapolis.
Responsibilities include:
- Supporting community engagement and participation
- Assistingwith outings such as:
- Community events and activities
- Assistingwith mobility needs, including pushing a wheelchair and supporting safe community navigation
- Helping organize paperwork, schedules, appointments, and personal documents
- Assistingwith identifying community resources, activities, and opportunities
- Encouraging independence, self-advocacy, and social connection
- Providing prompting, encouragement, and organizational support
Lead IHS Support & Administrative Responsibilities (Approximately 15 Hours/Week)
Serve as a lead support staff for severaladditionalIHS clients and workers.
Responsibilities include:
- Providing guidance and support to IHS workers, including family staff
- Assistingwith communication between staff, clients, and leadership
- Helping support training sessions alongside the Designated Coordinator and Designated Manager
- Providing coverage for staff call-outs or scheduling gaps
- Assistingwith organization and oversight of documentation and paperwork
- Conducting client and staff check-ins
- Meeting with clients in the community or home setting as needed
- Supporting staff with understanding goals, routines, boundaries, and best practices
- Helping ensure services remain organized, person-centered, and consistent
This role would serve as lead support forapproximately 34additionalclients.
Additional Client Support Roseville Area (510+ Hours/Week)
Additionalhours are availablesupportingclients in the Roseville area.
Responsibilities may include:
- Community outings and engagement
- Transportation to activities or physical therapy
- Encouragement with healthy routines and participation in the community
- Walking support and activity prompting
- Support with safe social media use and healthy habits
- Going to libraries, coffee shops, parks, and community activities
- Working in homes with pets, including a friendly and energetic dog
Qualifications
Preferred qualifications include:
- Experience working in Individualized Home Supports (IHS), disability services, mental health, or human services
- Strong organizational and communication skills
- Ability to work independently and problem-solve
- Comfortable supporting individuals with cognitive disabilities and mobility challenges
- Ability to mentor, guide, and support other staff
- Reliable transportation, valid drivers license, and willingness to transport oraccompanyclients in the community
- Comfortable working with diverse populations and personalities
- Ability to remain calm, supportive, patient, and encouraging
- Comfortable with community-based work and active support roles
