Work Schedule: 8 Hr (9:00:00 AM - 5:00:00 PM)

Pay Range: $40.05 - $61.28 per hour
Rush salaries are determined by many factors including, but not limited to, education, job-related experience and skills, as well as internal equity and industry specific market data. The pay range for each role reflects Rush's anticipated wage or salary reasonably expected to be offered for the position. Offers may vary depending on the circumstances of each case.
Summary:
This grant-funded program through the Chicago Department of Public Health (CDPH) supports a multidisciplinary street psychiatry team, serving individuals experiencing homelessness or housing insecurity with severe mental illness or other behavioral health needs. Work sites will include encampments, jails, hospitals, individuals' housing and other community organizations. The Community Health and Wellness Nurse is a nurse with knowledge in a specialized area of nursing who promotes and provides quality care to individuals, groups, and communities. The Community Health and Wellness Nurse supports and exemplifies Rush University Medical Center's mission, vision and values and acts in accordance with Rush policies and procedures.
Other information:
1. BSN or higher level of education
2. Current IL State RN licensure and CPR Certification
3. Interest in working with older adults
4. High energy and self-motivated with a strong focus on patient care and patient experience
5. Access to reliable transportation to and from sites
6. Bilingual in English/Spanish is a plus
Responsibilities:
1. Assesses health needs of individuals and the community
2. Plans and delivers nursing care for individuals, groups, and communities
3. Promotes continuity of care by working collaboratively with the interdisciplinary team at the assigned agency
4. Maintains, interprets, and evaluates health records and medical reports for clients
5. Develops and implements health education and prevention programs based on assessed needs
6. Implements various screening programs based on community needs
7. Utilizes community resources for client referral
8. Plans and delivers staff educational programs related to agency/client needs
9. Participates in agency program development and evaluation
10. Monitors compliance of regulated programs
11. Participates in quality improvement efforts
Skills:
1. Analytical ability is required

3. Interact effectively with others in difficult situations
4. Ability to use computer systems to enter or retrieve data
