RPM Care Manager

Location: Southfield, MI (On-Site) - EPIC Health has multiple office in Southfield. Employees assigned to Southfield will work primarily from one designated office based on operational needs.

Employment Type: Full-Time

Pay: $20-21/h
Schedule: Four 10-hour shifts per week (Monday–Thursday or Tuesday–Friday)
Reports To: Care Management Supervisor / Team Lead


Help Patients Stay Connected to Their Care

At EPIC Health, we believe proactive care leads to better outcomes. We are seeking a compassionate and detail-oriented RPM Care Manager to support patients enrolled in our Remote Patient Monitoring (RPM) program. This role is responsible for monitoring patient health data, engaging patients in their care, providing education, and coordinating timely clinical follow-up to improve outcomes for patients with chronic conditions.

Please Note: This is an on-site position. "RPM" refers to Remote Patient Monitoring, not remote work.


What You'll Do

Patient Monitoring & Engagement

  • Review daily RPM readings, including blood pressure, blood glucose, weight, pulse, oxygen saturation, and other assigned health metrics.
  • Monitor for missing readings, abnormal values, and concerning trends.
  • Contact patients regarding abnormal or missed readings, symptoms, medication adherence, and barriers to participation.
  • Encourage consistent patient engagement and program participation.

Care Coordination & Clinical Support

  • Follow established escalation protocols and promptly communicate concerning findings to providers or licensed clinical staff.
  • Coordinate care with physicians, care managers, medical assistants, pharmacies, and other members of the healthcare team.
  • Assist with enrolling eligible patients into the RPM program and educate them on device usage, program expectations, and participation requirements.
  • Identify patients who may benefit from additional care management services, provider follow-up, or community resources.

Patient Education

  • Educate patients on proper device use, measurement techniques, medication adherence, nutrition, activity, symptom monitoring, and chronic disease management.
  • Assist patients with troubleshooting RPM devices, applications, and connected monitoring technology.

Documentation & Compliance

  • Accurately document patient interactions, readings, interventions, education, and escalations within the electronic health record.
  • Verify patient consent, diagnoses, care plans, and device assignments are complete and compliant.
  • Support RPM device inventory, replacement, return, and tracking.
  • Maintain compliance with HIPAA, payer requirements, and organizational policies.

Quality & Performance

  • Meet productivity, documentation, and patient outreach expectations.
  • Participate in team huddles, quality audits, training, and continuous improvement initiatives.
  • Support a positive patient experience through responsive, compassionate communication.

Clinical Escalation Responsibilities

  • Confirm abnormal readings when appropriate.
  • Assess patients for associated symptoms.
  • Follow established clinical escalation pathways.
  • Notify licensed clinical personnel or providers when intervention is required.
  • Direct patients to emergency services when indicated by organizational protocols.
  • Document all escalations and patient instructions accurately.

The RPM Care Manager does not independently diagnose conditions, prescribe medications, or provide treatment outside the scope of their credentials.


What You Bring

Required Qualifications

  • High school diploma or GED required.
  • Healthcare experience as a Medical Assistant, Pharmacy Technician, Certified Nursing Assistant, Care Manager, Health Navigator, or similar role preferred.
  • Experience in primary care, chronic care management, patient outreach, population health, or remote patient monitoring preferred.
  • Knowledge of chronic conditions such as hypertension, diabetes, COPD, heart failure, obesity, and cardiovascular disease.
  • Strong computer skills and experience with electronic health records.
  • Excellent communication, organizational, and time management skills.
  • Ability to recognize abnormal readings and follow established escalation protocols.

Preferred Qualifications

  • Current healthcare certification or license.
  • Experience with Athenahealth, ChronicCareIQ, 3CX, or similar healthcare platforms.
  • Knowledge of Medicare, Medicaid, value-based care, or RPM programs.
