In this role, you will be a vital anchor for our healthcare team, assessing and monitoring all aspects of our individuals' medical and psychiatric care.
This role offers immense professional autonomy, a flexible schedule, and an escape from the relentless pressure of insurance-driven hospital metrics.
What You'll Do (Responsibilities)
  • Collaborative Care: Partner with community healthcare providers, nutrition services, and behavior specialists to deliver high-quality, holistic care.
  • Care Coordination: Oversee and coordinate all healthcare appointments, ensuring thorough and timely follow-ups.
  • Team Education: Train, empower, and delegate medication administration and nursing tasks to the Direct Support Team in accordance with state licensing and Mosaic policies.
  • Medical Record Management: Maintain, review, and audit medical records to monitor health concerns and document the delegation of nursing tasks.

Schedule & On-Call Structure
  • Primary Hours: Generally day hours.
  • On-Call Rotation: Every 3rd week (primarily phone-based for emergencies; in-person response is rare).
  • On-Call Support: You are never on an island-you will be backed by a strong support network, including a team of supervisors, QIDPs (Qs), and an RN Supervisor.

Job Requirements
  • Education: Must have an RN license, BSN preferred
  • Experience: At least one (1) year of clinical registered professional nursing experience within the last five years
  • Licensure: Active and unencumbered State Registered Nurse (RN) license.

Physical Demands
  • Frequent standing, walking, bending, reaching (overhead, shoulder, and below knee level).
  • Occasional kneeling, climbing, stooping, crouching, and balancing.
  • Ability to occasionally lift up to 25 lbs independently, push/pull up to 40 lbs of force, and safely transfer up to 50 lbs.
