RN Case Manager

Position Overview

The Registered Nurse Case Manager (RNCM) is experienced in nursing, and leads each patient’s hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff. The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.

Essential Functions and Skills
Patient Care

  • Provides care in accordance with the Nurse Practice Act and professional nursing standards.
  • Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history.
  • Develops, implements, and revises the Plan of Care based on ongoing patient evaluations.
  • Administers medications and treatments as prescribed in the physician’s plan of care.
  • Educates patients and families on self-care, medications, and diet as appropriate.
  • Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition.
  • Coordinates care with physicians, interdisciplinary teams, and community health partners.
  • Ensures a safe environment for patients and supports their activities of daily living.
  • Provides emotional support and counseling to patients and families.
  • Arrange for necessary medical equipment and supplies.
  • Ensures HIPAA compliance and patient confidentiality.

Additional Duties

  • Participates in on-call visits as required including evenings and weekends.
  • Attend interdisciplinary group meetings.
  • Supervises ancillary personnel and delegates responsibilities as needed.
  • Engage in professional development through training and continuing education.
  • Contributes to quality assessment and performance improvement activities.
  • Performs other duties as assigned.

Requirements/Qualifications

  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.
  • Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.
  • Ability to pass DHS background, consumer report, and drug test.
  • Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Preferred

  • 1+ years of recent experience in hospice/home care preferred.
  • 2+ years of Nursing experience, preferred
