We are currently seeking qualified candidates for a part time Registered Nurse (RN) Case Manager positionย in the South Austinย area.
Job Description:
ย  This is an out-patient position,ย that requires an independent and experienced RN.ย  The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family.ย  RN-Case Managerย main job duties include and are not limited to;ย admissions, assessments, discharges and pronouncements. ย 
Qualifications:
  • ย State of Texasย RN license - ย required
  • 1+ ย years'ย experience as a RN - preferred
  • Experience with Hospice or Home Health - preferred
  • Effective written and verbal communication
  • Ability to travelย  - must haveย reliable transportation
  • Clean background,ย drug screen, and driving record
