Job description

Community Behavioral Nursing Services is a family-owned and operated, accredited home health agency dedicated to empowering individuals with intellectual and developmental disabilities to live with dignity and respect. Unlike traditional home care roles, our nurses typically travel no more than fifteen miles between patient locations, allowing for a better work-life balance, and less time on the road. CBNS proudly serves the Stark, Medina, Cuyahoga, and Lorain Counties- and we are continuing to grow! We are currently seeking a dependable, self- motivated Registered Nurse to join our team!


Compensation:Starting at $70,000- $72,000 annually


Job Type:Full-Time Exempt


Schedule:The RN Case Manager completes visits in group home settings every Monday, Wednesday, and every other Friday in Lorain, Amherst, and Oberlin, Ohio. This position also includes one weekend assignment per month, and is in an on-call rotation.


Job Description:

The RN Case Manager is a hybrid, field nurse/manager role that offers a competitive salary. RN Case Managers oversee the long-term care for patients by planning, organizing, and directing home care services. This individual should have experience in nursing, with an emphasis on community health. The RN Case Manager meets the needs of individuals and families within their homes and/or communities, while also providing quality patient care based on the individual patient's care plans. The RN Case Manager acts as a liaison between CBNS Administrative and Clinical Staff, patients, families, and/or caregivers, while ensuring professional standards of care as established by CBNS policies and procedures.


Essential Job Functions and Responsibilities:

  • Complete assessments of patient to determine home care needs
  • Provide a complete physical assessment and history of current and previous illness(es) that corresponds with the physician's orders through assessment of patient's needs, condition, environment, etc.
  • Assisting the patient and the family and/or caregivers in the care planning process
  • Initiate appropriate preventative and rehabilitative nursing procedures
  • Administering medications and treatments as prescribed by physician
  • Responsible for the delivery of quality patient care in the home
  • Maintain communication with the Clinical Manager, CBNS Staff, and all other treatment team members
  • Care Conference participation as needed
  • Maintain a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conferences, and professional organizations
  • Participate in an on-call rotation with other RN Case Managers, as scheduled by CBNS
  • Ensure arrangements for equipment and other necessary items are available to patient and/or family/caregivers
  • Provide supervision and evaluation of LPN services as assigned by Clinical Manager
  • Perform OASIS assessments
  • On- Call for assigned territory Monday through Friday, and company-wide on- call every seven weeks


Qualifications:

  • Active, unencumbered Registered Nurse (RN) licensure in the State of Ohio
  • Minimum of one year's experience as a Registered Nurse (RN), preferably in a home care, clinical setting
  • Preferred experience in a supervisory role in acute and/or home health
  • CPR Certification
  • Valid Driver's License and Auto Insurance
