What Youโ€™ll Do
  • Patient Care & Case Management
  • Perform comprehensive nursing assessments and develop individualized plans of care
  • Coordinate and manage home health services in collaboration with physicians and care teams
  • Provide skilled nursing care, treatments, and patient education
  • Monitor patient progress and communicate changes in condition to physicians
  • Implement preventive, rehabilitative, and restorative nursing interventions
  • Identify and coordinate discharge planning needs
  • Care Coordination & Documentation
  • Maintain accurate, timely clinical documentation and patient records
