Pay: $42.00-$51.21/hour
Registered Nurse Case Manager Opportunity:
  • Full-time, permanent position
  • Monday-Friday, working 8:00AM-5:00PM
  • Working in either Home Health or in Hospice
  • Rotating on-call every holiday
  • The Registered Nurse Case Manager will collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans
  • the Registered Nurse Case Manager will coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions
Registered Nurse Case Manager Requirements
  • Registered Nurse License Required
  • Atleast one year of case management experience working in a clinical setting
