POSITION SUMMARY โ€“ RN Care Manager

POSITION DUTIES โ€“ Provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. Highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

MINIMUM REQUIRED QUALIFICATIONS โ€“

  • Nursing Diploma or ADN / ASN from an accredited school of Nursing
  • NC/compact RN license
  • 2 years acute care hospital case management experience
  • Care manager / discharge planning experience

PREFERRED QUALIFICATIONS โ€“

  • EPIC experience
  • Experience with educational/teaching hospital
  • BSN or MSN from an accredited school of Nursing

LENGTH OF ASSIGNMENT โ€“ 13 weeks

SHIFT / HOURS PER WEEK โ€“ 8:30am-5:00pm
