Description
American Traveler is seeking an RN Care Manager for an acute care setting with case management, discharge planning, and utilization review responsibilities, requiring a NC or compact RN license and 2+ years of RN experience.
Details
- Hospital-based Care Management department serving a 20-25 bed unit covering Cardiology, Nephrology, Trauma Surgery, Orthopedic Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, and Transplant, including ICU/Stepdown for these services
- Patient population consists of acutely ill patients with complex medical needs
- Day shift schedule, 8:30am-5:00pm, 40 hours per week, 5 days per week
- No weekend, holiday, or on-call requirements
- Uses Epic EMR for documentation and care planning
- Academic teaching hospital environment
Requirements
- Active RN license (NC or compact) required
- Minimum 2 years of healthcare experience as a Registered Nurse required
- Nursing Diploma, ADN, or ASN from an accredited nursing school required; BSN or MSN preferred
- Prior experience as a case manager, care manager, or discharge planner in an acute care hospital setting preferred
- Strong assessment and critical thinking skills required
- Epic EMR experience preferred
Additional Information
- Conduct comprehensive patient assessments and develop individualized plans of care documented in Epic
- Participate in daily Care Management Touchpoint meetings and CAPP meetings to review patient progression and discharge readiness
- Attend weekly Complex Care Meetings to collaborate on complex patient cases and identify trends
- Coordinate with Social Workers, Utilization Managers, and Care Management Assistants to address barriers to discharge and psychosocial concerns
- Identify expected discharge dates and monitor observation patients for appropriate care progression
- Educate patients and families on community resources, post-acute care options, and discharge plans
- Coordinate referrals to post-acute providers and identify required authorizations for post-discharge services
- Experience in educational or teaching hospital settings is highly beneficial
