Job Description: RMA Patient Navigator

RMA Patient Navigator Job Summary: Under general supervision of the Nursing Supervisor the Triage position performs care coordination and professional nursing functions to assess the severity of the patient’s health concerns using protocols and algorithms in a manner that provides patient dispositions to emergency care, clinic provider appointments, and triage nurse clinic services within the scope of the licensed practice. The Triage staff member will support providers in triaging all walk in patients.

Candidates should have a strong community health/public health orientation.

RMA Patient Navigator Essential Accountabilities:

  • The Triage staff member will be part of a care coordination team supporting general care processes alongside the providers.

  • Triage staff member will schedule/direct patient telephone calls, including walk-in patients requesting health care services.

  • Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.

  • Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.

  • Accurately documents symptoms/complaints, advice provided and patient/caller response. Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate healthcare.

  • Coordinate and schedule future, same-day, and walk-in appointments on the provider schedules.

  • Utilize triage protocols appropriately.

  • Monitor patient flow in clinic; ensure that patients receive all recommended services in a timely manner.

  • As needed, perform other nursing functions as assigned. Assist providers with special procedures as needed.

  • Assist with prescription refills and communications per protocol.

  • Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.

  • Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation, follow-up.

  • Completes other duties as assigned by Nursing director.

  • Track patients that “no show” for appointments.

  • Set up new patients for appointments and do all required new patient paperwork.

RMA Patient Navigator Knowledge/ Skills/ Abilities:

  • Must have knowledge of disease processes, normal growth, and development of all age groups.

  • Must possess community health knowledge.

  • Must operate with culturally sensitivity with respect to working with persons and groups of diverse racial, ethnic, economic, and social backgrounds.

  • General computer skills required. Experience with EHR systems.

  • Ability to maintain a positive attitude with changing conditions.

  • Ability to work in a fast-paced department and handle multiple tasks, work with interruptions, and deal effectively with confidential information.

  • Seeks out available resources to resolve problems

  • Possess strong verbal and writing communication skills.

  • Possess a professional and mature demeanor.

  • Ability to work effectively under pressure by prioritizing work activities.

  • Ability to function independently as well as the ability to work as part of a team.

RMA Patient Navigator Qualifications:

Education:

  • Minimum Required:

    • Minimum 2 years of healthcare experience

    • Certified Nurses aide/Medication aide/or Medical assistant

Experience:

  • Previous experience in rural health clinic preferred.

  • Triage experience of a least 1 year preferred.

Skills:

  • Proven leadership, physician relations and communication skills.

Licensure/Certification:

  • Currently CPR certified.

Environmental Surroundings: Located in an indoor area with frequent exposure to mild physical discomfort from dust, fumes, temperature, and noise. Examples: patient care providers and laboratory technicians.
