Rise & Shine is hiring a Certified Nursing Assistant (CNA) to provide direct, hands-on support to individuals with intellectual and developmental disabilities. Working under the clinical direction of a Registered Nurse or other licensed medical professional, you will support individuals with daily care needs, monitor changes in their condition, document services accurately, and communicate closely with nursing and program leadership.
What Youโll Do
- Provide respectful, hands-on care. Assist individuals with activities of daily living, including bathing, grooming, dressing, toileting, mobility, feeding, and personal hygiene while supporting their comfort, dignity, safety, independence, and personal choices.
- Monitor health and communicate concerns. Observe changes in physical condition, behavioral presentation, health status, safety, or care needs and report concerns promptly to the RN or supervisor. Assist with vital signs, basic health monitoring, and non-invasive care as directed by licensed nursing staff.
- Support safe mobility and daily routines. Assist with positioning, transfers, and mobility using proper body mechanics and established safety guidelines. Provide support during meals, appointments, community activities, and other person-centered routines as assigned.
- Follow individualized care plans. Provide support in accordance with Individual Support Plans, health care plans, nursing direction, agency policies, and other assigned care instructions.
- Document care accurately. Record care provided, observations, and required shift information in Therap or other designated systems within required timelines.
- Create a safe and supportive environment. Help maintain a clean, organized, and welcoming living environment while responding appropriately to urgent or emergency situations.
- Collaborate with the care team. Communicate respectfully with individuals, families, coworkers, nursing staff, program leadership, and outside providers while maintaining confidentiality and professional boundaries.

Requirements:
Minimum Requirements
- Must maintain active CNA certification in the State of Oregon.
- Knowledge of basic caregiving and resident-support practices.
- Ability to follow care plans, instructions, agency policies, and clinical direction.
- Strong interpersonal and communication skills.
- Ability to remain calm and responsive in high-stress or emergency situations.
- Basic computer skills; Therap experience preferred, but not required.
- Ability to work collaboratively as part of a multidisciplinary care team.
- Must pass and maintain all Oregon-mandated criminal background screenings and eligibility requirements applicable to the position.
- Must be able to remain awake, alert, and attentive while on shift.
- Must be flexible and willing to work at different program locations based on operational needs and staffing requirements.
Training and Certification Requirements
- Must successfully complete and maintain all trainings, certifications, competency requirements, and continuing education required by federal and state law, Oregon Administrative Rules, licensing entities, program protocols, and company policy.
- Must be physically capable of demonstrating and performing all required intervention techniques, support strategies, rescue procedures, emergency response protocols, and other duties associated with such trainings and certifications, as appropriate to the position and the needs of the individuals supported.
Physical and Work Requirements
- Must be able to lift up to 40 lbs. and perform physical support tasks such as bending, squatting, pushing, pulling, reaching, kneeling, crouching, transfer-related support, and assisting individuals as needed, within training, program protocols, and company safety guidelines.
- Must be able to stand and walk for extended periods of time and navigate stairs as required by program environments.
- Must be able to safely perform personal care, mobility support, household tasks, and community-based activities as required by individual support plans and program needs.
- Must be physically capable of responding appropriately in emergency situations.
- Must be able to perform the essential functions of the position with or without reasonable accommodation.

Coaching teaches people how to improve their performance. Training teaches people what to do. Culture teaches people how to treat each other.
