Physical Therapist โ€” Home Health | Flexible Full-Time, Part-Time & PRN Options

WellTech Partners is partnering exclusively with a five-star, clinician-led home health organization in the Bay Area โ€” and they're looking for a Physical Therapist who believes great outcomes start with genuine relationships.

This isn't a revolving-door caseload. It's a role built for a clinician who wants to practice at the top of their license, own their day, and see the direct impact of their work โ€” one patient at a time, right in their home.

Compensation & Schedule
  • Salary: $130,000 โ€“ $140,000 annually
  • Schedule: Monday โ€“ Friday, 9 am โ€“ 5 pm (Part time and PRN opportunities also available)
  • Visits per day: Approximately 5
  • Benefits: Comprehensive package including health, PTO, mileage reimbursement, and more
What You'll Be Doing

You'll serve as a key partner in helping patients restore mobility, reduce fall risk, and regain independence โ€” all from the comfort of their homes. Working across San Jose and surrounding areas, you'll support a geriatric population with diverse clinical needs, bringing your expertise directly to the people who need it most.

This is a field-based role where no two days look exactly alike โ€” and where your clinical judgment genuinely shapes patient outcomes.

What We're Looking For

Required:

  • Active California Physical Therapist license (in good standing)
  • Graduate degree in Physical Therapy (DPT preferred)
  • Experience managing a patient caseload independently
  • Strong communication and clinical decision-making skills
  • Reliable transportation for home visits
  • Home health experience

Preferred:

  • Experience with Medicare/OASIS documentation
  • Familiarity with home health EMR systems (e.g., WellSky/Kinnser)
  • Experience working with geriatric or medically complex patients
