SUMMARY
Responsible for assisting with the timely completion of accurate resident assessments, coordination of interdisciplinary conference and maintaining compliance with state and federal regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned Assist the MDS Coordinator with the following duties and responsibilities: Develop and maintain accurate individualized care plans with measurable goals using the designated format for long-term admissions. Assure that acute PCP has been completed within the first twenty-four hours. Maintain up-to-date care plans for all residents residing at the facility. Conduct MDS assessments using designated format as designated by the MDS Coordinator. Advise and effectively communicate the plan of care to nursing staff and ensure that the plan is being followed. Coordinate with multiple parties’ care plan conference schedules. Stay abreast of state and federal laws governing the care plan process and make modifications as deemed necessary. Must interact courteously, tactfully and politely with managers, co-workers, residents, vendors or associates to preserve productivity and moral.

Qualifications/Experience
Associates or Bachelor's degree in Nursing and licensed in the state of NC. Experience working in LTC, SNF is required. Must be licensed in the state of NC as a LPN/RN.
