Job description

Responsible for the scheduling and reconciling of all home care services.
Schedule is Monday - Friday, working 8:00am - 5:00pm.
Compensation: $22.85 - $23.99 an hour

Duties and Responsibilities
  • Creates and maintains participant schedules according to care plans and staff availability.
  • Maintains staffing schedule to ensure coverage for sick leave, vacation, and long-term leave.
  • Communicates routinely with contract agencies to maintain staffing and a long lasting relationship.
  • Assists with reconciling vendor invoices and delivering the services in the EMR.
  • Coordinates Durable Medical Equipment (DME) and delivers DME items.
  • Assist with semiannual home care assessments.
  • Contact care providers and clients regarding day-to-day changes.
  • On a rotational basis, participates in after hours and weekend calls to handle emergencies, scheduling issues, and client referrals/intakes.
  • Perform general office duties including but not limited to word processing, photocopies, files, shreds, sort/distribute mail, provide reception coverage, telephone services, etc.
  • Understand and adhere to established home.

Physical Demands
  • Ability to lift up to 30 pounds. Moving, lifting or pushing greater than 30 pounds should be done with assistance as appropriate.
  • Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
  • Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

Work Environment
  • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
  • Subject to unpleasant odors.
  • The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Education/Experience Requirements
Minimum Qualifications
  • Reliable transportation valid CA Driver's License, acceptable driving record, and vehicle insurance.
  • Must be able to work effectively in a team environment.
  • Only act within the scope of his or her authority to practice.
  • Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, be flexible, innovative, focus on results and travel regularly.
  • Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Meet a standardized set of competencies established by Central Valley PACE and approved by CMS before working independently.
  • Proficiency with Microsoft Suite and/or Google Suite, and scheduling software required.
  • Bilingual English / Spanish.

Education/Experience
  • High school diploma, or equivalent.
  • Current BLS CPR Card certified by American Heart Association.
  • Minimum two (2) years caregiver experience.
  • Minimum one (1) year administrative experience.
