Job description

Admissions, Registered Nurse (RN) Position

HopeHealth is seeking a Registered Nurse (RN) who is passionate about end-of-life care and an advocate for the seriously ill to provide hospice services to patients in their homes and long-term care facilities.

As a Hospice Admission RN, you will provide skilled assessments to determine patient eligibility for hospice services and complete admissions for eligible patients.

This is a per diem position that is not eligible for benefits.

What Will an Admissions, Registered Nurse (RN) Do?
  • Represents HopeHealth in assigned settings and is a subject matter expert in the hospice benefit.
  • Presents hospice to eligible patients and caregivers in a manner that highlights the benefits of the service in caring for individuals with a terminal diagnosis.
  • Performs initial assessment of patient and family to determine the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living. Applies knowledge of Local Coverage Determination training to communicate assessment to the hospice Medical Director.
  • Completes hospice admissions in compliance with Medicare/Medicaid and state regulations.
  • Creates individualized plan of care including documenting the Local Coverage Determination.
  • Uses interdisciplinary approach in developing plan of care, in accordance with organizational policies. As needed, initiates referrals to other disciplines.
  • Participates in weekly hospice interdisciplinary group (IDG) meetings to review patients on service as needed.
  • Documents in EMR at point of care and meets department expectations for timely completion of documentation.
  • Ensures that patient information regarding payer source is current and correct. Works with referral source to obtain necessary insurance information and coverage.
  • Completes other duties as assigned, including but not limited to, field evaluations/admissions, triage visits and GIP coverage at adjacent sites.
  • Participates in weekend rotation and holiday coverage for HopeHealth.
What Are the Qualifications for an Admissions, Registered Nurse (RN)?

  • Licensed as a Registered Nurse in Rhode Island and Massachusetts
  • Minimum of 1-3 years of clinical experience
  • Excellent nursing assessment, clinical and communication skills
