Stonington Institute is committed to providing quality substance abuse treatment options for adultsin Southeastern Connecticut and surrounding areas. We offermedical detox aswell as residential addictions treatment for active duty and retired military service members in our North Stonington location. Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is conveniently located across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnight accommodations, full-meal service, indoor/outdoor recreation options, a workout room, and computer center for all clients. We accept private insurance, state-funded plans and self-pay arrangements.  

Essential Responsibilities:

Under the direction of a Registered Nurse, the Licensed Practical Nurse (LPN) contributes to the care of clients by collecting, reporting and recording objective and subjective data in an accurate and timely manner, participating in the development of a strategy of care in consultation with other nursing personnel, and participating in the assisting, delegating, and giving of directed care.

Job Duties and Responsibilities:

· Reconciles medication and performs health screenings.

· Accurately notes, transcribes, and implements Physician’s orders and faxes prescriptions to pharmacy.

· Participates in the assisting, delegating, and giving of directed care.

· Participates in the development of a strategy of care in consultation with other nursing personnel.

· Evaluates client’s medical condition.

· Inventories current medications and updates medication reconciliation form in chart.

· Reports to staff on medical conditions of clients and any medication changes.

· Ensures that appropriate medications and supplies are available and orders medications from the pharmacy as needed.

· Educates clients on medications, medication management, health maintenance, substance abuse, and other applicable topics.

· Monitors the medical aspects of treatment of persons afflicted with substance dependence and behavioral health conditions.
