Be a part of the new Solano THRIVE Center, "Transforming Hope into Resilience, Inspiration, Vision, and Empowerment!"

SCHEDULE
This is a full-time, non-exempt position. The THRIVE Center is designed to meet the needs of students and families during after-school and evening hours; therefore, this position will primarily work an afternoon-to-evening schedule, with one daytime shift each week to support alternate family schedules, partner meetings, planning activities, and community engagement efforts.
At this time, there is a minimum expectation of one Saturday per month. As the THRIVE Center grows and service partnerships expand, a more regular Saturday schedule may be implemented based on participant and community needs.
JOB SUMMARY
The Family Resource Navigator serves as the primary point of contact for participants accessing services through the THRIVE Center. This position plays a critical role in creating a welcoming and supportive environment while helping children, youth, and families identify their needs, access resources, and successfully connect with services.
Working closely with the Program Coordinator, community partners, and participating families, the Family Resource Navigator will conduct intakes, coordinate referrals, provide resource navigation, and support participants throughout their service journey. This position will help ensure that families experience the THRIVE Center as a trusted, responsive, and accessible resource where support is coordinated across multiple systems and providers.
The Family Resource Navigator will also support data collection, participant follow-up, community events, and continuous improvement efforts while helping maintain a welcoming "no wrong door" environment for all who visit the Center.
PROGRAM DESCRIPTION
We are seeking a compassionate, organized, and relationship-centered professional to support Bay Area Community Resources' (BACR) launch and operation of the THRIVE Center, an innovative community hub designed to make support simpler, warmer, and more accessible for children, youth, and families.
THRIVE will integrate educational support, behavioral health care, basic needs assistance, and peer-to-peer connection under a single, trusted roof. Its mission is to make help simpler, warmer, and faster for families and their children. Various county departments and community-based organizations will sustain THRIVE as a coordinated, one-stop community center where students and their caregivers can access the full continuum of support they need to thrive during and outside of standard school hours and throughout the academic year.
The THRIVE Center is being developed through a collaborative partnership among BACR, community organizations, public agencies, schools, and local leaders committed to improving access to services and strengthening outcomes for children, youth, and families. As the operator of the THRIVE Center, BACR will coordinate daily operations and partner collaboration to help ensure families experience seamless access to services, resources, and support. The Center will serve as a welcoming hub where families can access support, build connections, and navigate services through a coordinated and family-centered approach.
ESSENTIAL DUTIES AND RESPONSIBILITIES
• Serve as the primary point of contact for participants, creating a welcoming, inclusive, and family-centered environment that reflects the THRIVE Center's "no wrong door" approach
• Conduct intake assessments, eligibility screenings, consent processes, and participant orientation activities that introduce families to THRIVE Center services, supports, and family-centered approach in a trauma-informed, healing-centered, and culturally responsive manner.
• Provide resource navigation, information, referrals, and ongoing support to participants based on identified strengths, needs, and goals
• Coordinate referrals and warm handoffs to co-located partners, community providers, and external service systems, including follow-up to support successful service connections and reduce barriers
• Receive and triage referrals from schools, community-based organizations, and partner agencies, coordinating with the Program Coordinator when complex or multi-system support is needed
• Maintain accurate participant records, referral information, service data, and outcome measures while ensuring compliance with program requirements and confidentiality standards
• Support participant surveys, feedback collection, reporting activities, and continuous improvement efforts
• Help maintain a welcoming, organized, and well-functioning Center environment, including participant engagement areas, resource materials, and basic needs supplies
• Support outreach activities, workshops, community events, and partner gatherings while collaborating with the Program Coordinator and community partners to support coordinated service delivery and positive participant outcomes
QUALIFICATIONS AND/OR EDUCATION
• Associate degree or higher in Human Services, Social Work, Psychology, Child Development, Public Health, Education, Counseling, or a related field; or an equivalent combination of education, training, and experience
• Two years or more of experience working with children, youth, families, community members, or vulnerable populations in a community-based, educational, healthcare, or social services setting
• Experience conducting intakes, providing resource navigation, case coordination, family support, customer service, or other participant-facing services
• Strong interpersonal, communication, and relationship-building skills, with the ability to build trust and foster an inclusive and welcoming environment for individuals and families from diverse backgrounds
• Strong organizational skills, attention to detail, and ability to manage multiple priorities while maintaining accurate documentation
• Experience using databases, case management systems, or data collection platforms
