Mission 1:27's Mission: We serve people to glorify God and see lives transformed in loving obedience to James 1:27.


Position Purpose: To provide proactive care, connection, and support for under-resourced families and vulnerableย 

seniors - helping them thrive, stay connected, and maintain independence while assisting Community teams with resident needs.


What You'll Do:

1. Collaborate with both the Mission 1:27 team and the Walton Community team to assess resident and family needs and help provide short-term assistance, supportive services, or ongoing support in certain at-risk situations.ย 

2. Serve as a knowledgeable resource regarding community-based programs, services, and opportunities available to residents, families, and team members.ย 

3. Provide support, guidance, and consultation for residents and families experiencing complex or challenging situations.ย 

4. Help residents and families access in-home support, community resources, and other services that promote stability, independence, and overall well-being.ย 

5. Manage referrals and maintain detailed, accurate documentation related to resident services and support provided.ย 

6. Lead or facilitate support groups, workshops, or connection opportunities for residents.ย 

7. Call or make in-home visits to residents or families experiencing illness, hardship, or crisis situations.ย 

8. Call or make in-home visits to residents or families identified as needing additional support or in at-risk circumstances.ย 

9. Participate in outreach efforts to encourage resident engagement in community events, programs, and activities.ย 

10. Connect residents and families to counseling, mental health, or supportive care services, as appropriate.ย 

11. Meet regularly with Community Managers and Lifestyle Coordinator to ensure strong communication, collaboration, and coordinated resident support.ย 

12. Participate in training activities and meetings with the Legacy team and the greater Mission 1:27 team.ย 

13. Stay engaged in the broader community to remain informed about relevant programs, partnerships, and resources that may benefit residents and families.ย 

14. Serve as an ambassador for Mission 1:27 by promoting its vision, values, and services to residents, families, and guests.ย 

15. Follow all Mission 1:27 and Walton Communities policies, procedures, and professional standards.ย 

Requirements

What You Bring:ย 

Passion for supporting residents, seniors and families during difficult or transitional timesย 

Knowledge of community resources and supportive services available to children, families, single parents, adults, and senior adultsย 

Energetic, creative, personable, compassionate, and resourcefulย 

Bachelor's degree in Social Services, Human Services, Psychology, Education, Counseling, or a related field preferredย 

Experience in case management, social work, resident services, family support, or related community-based workย 

A positive attitude and a strong desire to serve others with excellence, dignity, and compassionย 

Self-motivated, dependable, and highly organizedย 

Excellent verbal and written communication skillsย 

Computer proficiency, including Microsoft Word, Excel, and Outlookย 

Agreement with and adherence to Mission 1:27's Statement of Faith and Qualificationsย 

Spiritually mature with active involvement in a local churchย
