Job description

Position Overview
Seeking a skilled and independent Registered Nurse (RN) to provide high-quality home health care services across multiple communities in Massachusetts. This role involves managing patient care, completing OASIS assessments, and coordinating with the interdisciplinary team to ensure optimal patient outcomes. The ideal candidate is autonomous, adaptable, and experienced in post-acute or assisted living patient populations.
Schedule & Work Details
  • Location: Arlington, Belmont, Lexington, Lincoln, Sudbury, Waltham, Watertown, Wayland, Weston, Winchester
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 32 hours
  • On-call/Weekend Requirement: Yes
  • Pay Rate: $51.00 to $53.00 per hour
  • Mileage Reimbursement: Yes, between patient visits from first patient to last patient
Key Responsibilities
  • Initiate, develop, implement, and revise individualized patient care plans in collaboration with physicians and care team members
  • Provide skilled nursing care in accordance with the plan of care and infection control standards
  • Perform comprehensive patient assessments including OASIS documentation and medication reconciliation
  • Supervise and support LPNs and home health aides, including conducting required supervisory visits
  • Assign and delegate tasks in accordance with state and federal guidelines
  • Deliver direct and indirect patient care while ensuring coordination across the healthcare team
  • Accurately and thoroughly document patient visits in a timely manner in accordance with policy and payer requirements
  • Ensure documentation is completed and synced within required timeframes
Required Qualifications
  • Active Massachusetts Registered Nurse (RN) license
  • Strong clinical assessment and documentation skills
  • Ability to manage a caseload independently
Preferred Qualifications
  • Ability to work autonomously and adapt quickly
  • Strong onboarding and learning agility
  • Experience in nursing home care, post-acute care, or assisted living settings
  • Excellent communication and organizational skills
  • Experience coordinating care across multidisciplinary teams
