Key Responsibilities

• Provide individualized mental health assessments and develop tailored treatment plans for clients experiencing significant emotional disorders and mental illnesses.
• Engage actively in interdisciplinary team collaborations to ensure cohesive care approaches and seamless client experiences.
• Utilize evidence-based practices to conduct brief screenings, assessments, and interventions, adapting your methodologies to meet diverse cultural and personal needs.
• Create and implement integrated care plans, facilitating access to essential services and resources for clients, families, and caregivers.
• Carry a caseload consisting of individual, family, and group therapy sessions, focusing on crisis intervention and client empowerment strategies.
• Document all client interactions and maintain accurate reports, ensuring compliance with ethical standards and regulatory requirements.
• Participate in on-call coverage as needed, providing crisis management during and after regular program hours.
• Contribute to quality improvement initiatives by participating in committee work aimed at enhancing service delivery.

Patient Population / Client Focus

The focus of this role is on clients within our hospice program, who are often navigating complex emotional and mental health challenges associated with end-of-life care. You will work empathetically to support both patients and their families, addressing grief, anxiety, and existential concerns while ensuring the provision of compassionate care. Understanding the importance of individualized approaches, you will empower clients by fostering resilience and facilitating connections to community resources.

Qualifications

• A Master's degree in Social Work (MSW) or a related field is required.
• Active LCSW licensure in the state of Connecticut is mandatory.
• Demonstrated knowledge of mental health assessments, diagnosis, and treatment modalities specific to hospice care.
• Exceptional interpersonal and communication skills, both verbal and written, with an ability to build rapport with a diverse client base.
• Strong organizational skills with the flexibility to adapt to changing circumstances and client needs.

Preferred Experience

• A minimum of two years of relevant clinical experience, particularly within hospice or palliative care settings.
• Bilingual proficiency in English and Spanish is highly desirable, enhancing communication and service delivery for our diverse client population.

• The competitive annual salary offered ranges from $80,000 to $100,000, commensurate with experience.
