Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our health center team on a full-time basis in a remote capacity. The ideal candidate will play a vital role in providing comprehensive behavioral health services aligned with our organization's mission to enhance the well-being of our community members. This position offers a competitive salary ranging from USD 90,000 to 100,000 annually.

Key Responsibilities
  • Deliver clinical behavioral health services that align with CHDC's mission, uphold quality standards, and enhance patient satisfaction and outcomes.
  • Assist mental health consumers in navigating both physical and behavioral healthcare systems, ensuring an integrated experience that optimizes care continuity.
  • Provide education to individuals on available health resources and services, promoting disease prevention and overall wellness.
  • Engage actively in multidisciplinary treatment team meetings, contributing insights and collaborating on care strategies.
  • Report all crisis situations and unusual incidents to the Medical Director promptly, with immediate escalation of psychiatric or medical emergencies to the Chief Medical Officer.
  • Maintain thorough documentation within clinical records, ensuring accuracy and compliance with regulatory standards.
  • Participate in quality improvement initiatives to enhance service delivery and patient outcomes, attending related meetings and training sessions.
  • Utilize evidence-based screening tools to identify healthcare needs, managing assessments and reporting outcomes rigorously.
  • Collaborate with the Center of Excellence Team to provide specialized care for patients facing Opioid Use Disorder.
Patient Population / Client Focus

The LCSW will support a diverse patient demographic within Upper Darby and surrounding areas, including persons from varying cultural backgrounds and socio-economic statuses. The focus will be on promoting mental health wellness, supporting those with chronic conditions, and addressing the complexities of substance use issues, particularly opioid dependence. Understanding the unique challenges faced by our community is crucial in tailoring effective interventions and support.

Qualifications
  • A Master’s degree in Social Work from an accredited institution, holding a current Licensed Clinical Social Worker (LCSW) credential.
  • A minimum of 2 years of clinical social work experience, with a strong preference for candidates with backgrounds in healthcare settings, particularly within primary or specialty care.
  • Excellent interpersonal skills, demonstrating the ability to collaborate effectively with a multidisciplinary team, set priorities, and manage caseloads efficiently.
