• Conduct comprehensive clinical assessments for patients to identify their mental health needs accurately, developing individualized treatment plans that employ evidence-based interventions.
• Administer therapeutic interventions such as cognitive-behavioral therapy (CBT) and other solution-focused modalities tailored to the unique circumstances of each patient.
• Provide ongoing support and therapy, helping patients navigate their emotional landscape and equipping them with effective coping strategies.
• Maintain detailed and accurate clinical records in compliance with HIPAA regulations and best practice standards.
• Work collaboratively with an interdisciplinary team, including physicians, nurses, and other mental health professionals to ensure comprehensive care delivery.
• Facilitate case consultations and participate in peer review meetings to enhance the quality of care and share best practices within the team.
• Educate patients and their families about mental health conditions, treatment options, and available resources, fostering a supportive environment for recovery.
• Continuously update personal knowledge of mental health research and advancements to enhance the effectiveness of treatment plans and patient outcomes.

In this role, you will engage with a diverse range of patients dealing with complex mental health issues related to end-of-life care, chronic illness, and various emotional disorders. You will provide crucial support to patients and their families who are navigating the often difficult emotions associated with hospice care, helping them to find comfort, closure, and understanding during challenging times.

• A Master’s degree in Social Work from an accredited institution is required.
• Must possess a valid LCSW license in the state of Virginia.
• A minimum of 3 years of relevant clinical experience in providing psychotherapy, particularly in a hospice or mental health setting.
• Demonstrated proficiency in solution-focused psychotherapy techniques and a commitment to holistic patient care.
• Strong communication and problem-solving skills, essential for empowering patients and effectively collaborating with the care team.
• A capacity to thrive in a collegial environment that values teamwork and shared success.

• Prior experience in hospice care or palliative settings is preferred but not mandatory.
• Familiarity with electronic health records (EHR) systems and telehealth practices.

• Annual salary ranges from USD 80,000 to 100,000, commensurate with experience and qualifications.
