This position is entirely remote, granting you flexibility while maintaining the structure of a full-time role. • Conduct thorough behavioral health diagnostics and crisis evaluations to assess patients’ needs and determine appropriate interventions.
• Utilize a range of standardized cognitive and behavioral assessment tools to form accurate and comprehensive patient profiles.
• Deliver ongoing psychotherapy to individuals and groups, fostering therapeutic relationships that promote healing and resilience.
• Determine the type and duration of services required for each patient, tailoring interventions to meet specific needs.
• Provide tele-presentation and telephone support services, ensuring patients and their families receive timely assistance.
• Coordinate care by collaborating with primary care providers, specialists, and home health aides to ensure a holistic approach to patient wellness.
• Offer case management support that empowers patients and caregivers through their recovery journey.

Our primary focus is on serving adults dealing with various mental health challenges, including substance use disorders, anxiety, depression, and trauma. You will work closely with diverse patient demographics, understanding their unique circumstances and delivering support that resonates with their individual experiences. Your work will significantly contribute to improving the quality of life for patients who are often at a vulnerable point, helping them navigate their recovery paths.

• A current, unrestricted state license as a Licensed Clinical Social Worker (LCSW).
• A Bachelor's Degree in Social Work or a related field.
• Demonstrated knowledge and competency in the assessment, diagnosis, and treatment of behavioral health issues.
• A passion for developing long-term care relationships and providing compassionate support to patients and their families.

• Experience in psychotherapy and a strong understanding of various therapeutic modalities.
• Adept at working within interdisciplinary teams to provide comprehensive care.
• Strong organizational skills and the ability to prioritize in a fast-paced, dynamic environment.
• Comfort and proficiency in using telehealth and other digital communication tools to engage with patients.
