This full-time position offers a remote work setting, allowing you to maintain a healthy work-life balance while providing essential support to patients in hospice care. You will operate from a digital platform that facilitates secure communication with clients and interdisciplinary team members, enabling effective collaboration and documentation of client interactions. Working hours are Monday through Friday, from 9:00 AM to 5:00 PM, ensuring predictable scheduling while providing freedom to manage your work environment.

• Conduct comprehensive virtual psychosocial assessments to evaluate clients' emotional, social, and psychological strengths and needs.
• Provide evidence-based therapy and counseling services to individuals and families, adapting approaches specifically for those facing end-of-life transitions.
• Collaborate with physicians, nurses, and healthcare providers to develop integrated care plans that promote holistic wellness for patients.
• Advocate for clients by connecting them to vital community resources that assist in their care and quality of life.
• Maintain detailed documentation of client interactions, treatment progress, and outcomes in electronic health records, ensuring compliance with organizational and regulatory standards.
• Engage in ongoing team meetings and case conferences to share insights and enhance clinical skills through continuing education and professional development opportunities.

Your primary focus will be on clients receiving hospice care, a population that requires sensitivity and expertise in addressing complex emotional responses related to terminal illnesses. You will facilitate discussions about grief, loss, and transition, ensuring that patients feel supported and heard during this crucial time in their lives.

• Master's degree in Social Work from an accredited program is required.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
• A minimum of two years of post-licensure experience in providing clinical social work services, specifically in mental health or hospice settings is preferred.
• Strong proficiency in conducting diagnostic assessments and developing tailored treatment plans that address unique client needs.
• Exceptional interpersonal and communication skills, with a focus on building rapport with diverse populations, including patients and their families.
• A commitment to cultural competence, social justice, and ethical practices in all interactions.

• Prior experience providing therapy in a hospice or palliative care setting is strongly preferred.
• Familiarity with digital healthcare tools and telehealth platforms is advantageous for remote therapy sessions.
• Experience in leading support groups or workshops related to grief, loss, or mental wellness is a plus.
