Work Environment

Key Responsibilities

• Collaborate closely with a multidisciplinary team, including psychiatrists, nurses, and other social workers, to identify and address the unique needs of each client.
• Develop and implement individualized treatment plans that are informed by thorough assessments and ongoing evaluations, ensuring alignment with best practices in the field of social work.
• Engage in regular monitoring of client progress, adjusting treatment plans as necessary to optimize outcomes.
• Provide education and resources on mental health issues, coping strategies, and available community services to clients and their families.
• Maintain accurate and up-to-date clinical documentation in accordance with organizational policies and state regulations.
• Participate in team meetings and case conferences to share insights and contribute to collective problem-solving.
• Stay informed about trends, advancements, and methodologies in social work, particularly regarding mental health, to enrich professional development and service delivery.

Patient Population / Client Focus

This role primarily focuses on serving a diverse clientele, including individuals from various age groups and backgrounds, with an emphasis on elderly populations. You will address issues ranging from mental health disorders to life transitions, ensuring that each client receives personalized care that acknowledges their unique experiences and challenges.

Qualifications

• A Bachelor's degree in Social Work is required, with an active state license as a Licensed Clinical Social Worker (LCSW).
• Minimum of 2 years of professional experience in clinical social work, particularly with elderly populations or similar demographics.
• Strong clinical skills, including assessments, diagnoses, and therapeutic interventions.
• Excellent communication skills, capable of fostering positive relationships with both clients and team members.
• Ability to work independently while being a proactive and collaborative team member.

Preferred Experience

• Experience with telehealth platforms and remote care delivery is a plus.
• Familiarity with evidence-based therapeutic modalities, such as cognitive-behavioral therapy (CBT) and supportive counseling.
• Previous involvement in community outreach programs or initiatives designed to promote mental health awareness.

Compensation and Benefits

We offer a competitive salary ranging from $90,000 to $110,000 annually, alongside a comprehensive benefits package. This includes:
