This position is fully remote, providing you with flexibility in your work schedule while allowing you to serve clients from the comfort of your home office. • Conduct comprehensive psychosocial assessments of seniors, integrating their emotional, mental, and social factors to inform care decisions.
• Collaborate closely with clients and their families to create tailored care plans that align with their specific needs, goals, and preferences.
• Provide counseling and emotional support, assisting seniors and their families with life transitions, such as bereavement, relocation, or health changes.
• Act as a strong advocate for clients, ensuring they have access to necessary community resources, services, and support systems.
• Work within interdisciplinary teams composed of healthcare professionals, caregivers, and nurses to deliver holistic care strategies.
• Regularly monitor clients' living situations and well-being through scheduled check-ins, home visits, and ongoing assessment of any safety concerns.
• Facilitate effective communication among family members to resolve conflicts and misunderstandings regarding the care of seniors.
• Educate clients and their families about available community resources, social engagement opportunities, and coping strategies to improve overall quality of life.
• Maintain detailed documentation of all client interactions, assessments, care plans, and progress notes in compliance with ethical standards and practices.
• Stay updated on current trends and regulations in social work, actively participating in training and professional development opportunities.

Your primary client base will consist of seniors facing various psychosocial challenges, requiring a nuanced understanding of aging-related issues. You will interact with clients who may be experiencing cognitive decline, health complications, or significant life changes, and it's paramount that you exhibit patience, empathy, and a depth of understanding when addressing their needs.

• A Master’s Degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Master Social Worker (LMSW) in Florida.
• A minimum of two years of hands-on experience in a social work capacity, particularly within community health or geriatric settings.
• In-depth knowledge of the social work domain, particularly related to issues affecting the elderly population and available community aids.
• Demonstrated proficiency in assessment, counseling, and problem-solving techniques that cater to diverse client needs.
• Adept at establishing rapport and trust with seniors and their families, showcasing strong interpersonal skills.
• Proficient in utilizing electronic health record systems and other software applicable in client management and reporting.

• Additional experience in a health care setting, particularly with interdisciplinary teams.
• Familiarity with community resources specific to senior care in Florida.
• Experience delivering culturally competent social work services to a diverse clientele.
