Work Environment

Key Responsibilities
  • Conduct psychosocial assessments for diverse clients, including individuals, adolescents, families, and groups, identifying their specific psychological needs and developing tailored treatment plans.
  • Perform initial comprehensive assessments to evaluate patient understanding of their situations and triage care effectively based on urgency and clinical importance.
  • Engage in regular interdisciplinary meetings, including patient rounds, to discuss treatment strategies and collaborate with other healthcare professionals, ensuring a holistic approach to patient care.
  • Provide high-quality case management, facilitating communication between patients and community resources, and coordinating care with hospital and community professionals.
  • Ensure timely and accurate documentation that complies with departmental standards, maintaining ethical practices and safeguarding patient confidentiality.

Qualifications
