Key Responsibilities

• Conduct virtual individual and group therapy sessions tailored to the unique needs of clients facing terminal illnesses.
• Develop, implement, and adjust treatment plans grounded in comprehensive diagnostic evaluations and the ongoing assessment of client progress.
• Collaborate closely with a multidisciplinary remote team of healthcare professionals to coordinate holistic care, ensuring all aspects of a client's well-being are addressed.
• Maintain thorough, confidential documentation of client interactions, treatment plans, and progress notes using our secure electronic medical record system.
• Provide crisis intervention and stabilize situations for clients in need, ensuring they have access to the appropriate resources and support.
• Facilitate communication with family members, offering support and psychoeducation to help them understand the care process and cope with emotional challenges.

Patient Population / Client Focus

As a Clinical Social Worker in a hospice environment, you will primarily work with clients who are experiencing advanced stages of chronic illness or end-of-life issues. This role requires a deep understanding of grief, bereavement, and palliative care to effectively support not only the clients but also their families. Your ability to provide empathetic, culturally competent care will be instrumental in helping clients maintain dignity and peace during their final stages of life.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) credential.
• Minimum of two years of post-licensure clinical experience, preferably in a hospice, palliative care, or oncology setting.
• Demonstrated proficiency in conducting diagnostic evaluations and creating effective treatment plans.

Compensation and Benefits
